Population Health Management Framework evidences positive outcomes for blood pressure and falls.

Recently the Blood Pressure @ Home Monitoring project implemented the Population Health Management Framework (PHMF) 'Logic Model' to develop a comprehensive PHMF evaluation to understand if patients who were already diagnosed with hypertension or at risk of CVD within three three Primary Care Networks (PCN's), Chiswick, Hounslow Heath and Feltham & Bedfont were now controlling their blood pressure (BP).

Hounslow Falls Prevention and Bone Service

The new Hounslow Falls Prevention and Bone Service applied the Population Health Management Framework to help develop the first of its kind operating model designed to prevent avoidable falls and subsequent admissions to hospital for people aged 65-79. As data had evidenced that Hounslow had the highest falls incidence of all London Boroughs. Read how the framework supported research, project planning and evaluation here.
