Our History

Our timeline

  1. 2018 - Hounslow health and care providers from Hounslow and Richmond Community Healthcare, West London Trust, West Middlesex University Hospital, Hounslow Council, GP Consortium and the Voluntary Care Sector began meeting monthly as part of the Sustainability and Transformation Plan (STP) Implementation Group with the aim to explore opportunities for improving and integrating care locally.

  2. 2019 - As part of national policy, STPs were disbanded, and became the Hounslow Integrated Care Partnership (ICP). The ICP branded itself for residents as Hounslow Health and Care, working to agree approaches for developing integrated care in Hounslow.

  3. 2020 - The ICP’s agreed vision was, “working together to support Hounslow communities to live healthy, happy, connected lives.” Following the pandemic, the ICP work programme focussed on a number of workstreams designed to recover from Covid-19.

  4. 2022 - The name was changed from the Hounslow Integrated Care Partnership (ICP) to the Hounslow Borough Based Partnership (BBP) because of national policy changes. Hounslow BBP is one of eight BBPs, or ‘places’ that make up the NW London Integrated Care System (ICS).
