2019 - As part of national policy, STPs were disbanded, and became the Hounslow Integrated Care Partnership (ICP). The ICP branded itself for residents as Hounslow Health and Care, working to agree approaches for developing integrated care in Hounslow.

2020 - The ICP’s agreed vision was, “working together to support Hounslow communities to live healthy, happy, connected lives.” Following the pandemic, the ICP work programme focussed on a number of workstreams designed to recover from Covid-19.

2022 - The name was changed from the Hounslow Integrated Care Partnership (ICP) to the Hounslow Borough Based Partnership (BBP) because of national policy changes. Hounslow BBP is one of eight BBPs, or ‘places’ that make up the NW London Integrated Care System (ICS).
