Mental Health

Purpose

To oversee and scrutinise the delivery of all Mental Health (MH) transformation work in Hounslow as an integrated partnership, ensuring a focus on outcomes and maintaining alignment with the Hounslow ICP, NWL ICS MH & Wellbeing Programme, and the last year of delivery of the Long-Term Plan.;

1) Integration into primary care; covering 

  • MINT; the delivery of the model and alignment to PCNs, including flow between providers 
  • MHARRS; the delivery and embedding of roles, plus additional roles, alongside joint responsibility of MH support, care and treatment (step up/step down) between providers
  • Develop links between primary care and supported Housing

2) Link workers across system

The system delivery and coordination of link workers and ensuring seamless care provision and a standardised way of supporting local residence, including knowledge sharing and triangulating care to avoid duplication in provision/resources. 

3) VCSE Programmes

The borough-based delivery of mental health provision across all ages groups to local marginalised populations through the third sector. 

Focusing on increasing access, improving support in accessing services and knowledge about mental health (such as reducing stigma), increasing care provisions options for mild to complex needs and improving pathways between statutory and non-statutory providers.

4) Older Adults Crisis Interventions

To monitor the delivery and support pathways across the system of crisis care for older adults

5) MH Comorbidities; covering PH interface 

  • Dual diagnosis (substance misuse)
  • Integration with PH

6) Thrive and Learning Disability & Autism in adults; 

Oversee and support the delivery of the iThrive model 5 areas and 7 priorities within the borough based partnership group. 

Integration into primary care; covering 

  • MINT; the delivery of the model focusing on flow between providers; measured through S1 time between referral to assessment
  • MHARRS; the delivery and embedding of additional roles, alongside joint responsibility of MH support, care and treatment (step up/step down) between providers; measured through number of stepped down care via S1 or prevention of escalation to MINT via S1.
  • Strengthening existing relationships improved PCN interface meetings; reduced volume of flow held at MINT level
  • Links between primary care and supported Housing; measured through contacts/referrals
  • IAPT interface

VCSE Programmes

Easier access to VCSE provision via GP referrals; improve awareness and signposting/referrals

  • Increasing number of people accessing various funded provision; data collection of referral from and total referrals
  • increasing care provisions options for mild to complex needs; measuring number of SU accessing various provisions
  • Improved care outcomes; capture of Outcome measure pre and post interventions
  • Increased number of referrals to VCSE providers through MINT; data collection of referrals
  • Improved pathways links between statutory and non-statutory providers; measure through reporting/ step downs or via MHARRS

MH Comorbidities

  • Dual diagnosis (substance misuse)
  • PH Checks- uptake and score measured as a system 

Thrive and Learning Disability & Autism in adults

  • To have regular updates on the delivery plans and changes to improve LDA care within pathways across the system.
