Last week, we were delighted to welcome senior leaders from Singapore’s National Healthcare Group (NHG) to meet the newly launched Hounslow Care Together team (the new integrated neighbourhood health and care model), and senior leaders from the BBP.
The delegation, led by Professor Eugene Fidelis Soh, Deputy CEO (Population Health), and Professor Jonty Heaversedge, Clinical Director and CCIO (Population Health), visited the Heart of Hounslow NHS Health Centre to learn more about how Hounslow council, NHS and local voluntary partners in Hounslow are working together to support residents with frailty and complex needs.
Colleagues shared insights into the co-design and delivery of Hounslow Care Together, a model that brings professionals together in small, co-located neighbourhood teams to deliver proactive, joined-up care closer to home.
The visit provided a valuable opportunity for mutual learning between two systems facing similar challenges: how to shift from reactive to preventative care, strengthen relationships, and design services around people rather than organisations.
