Hounslow’s Health Outreach Team (HOT) was highly commended at this year’s prestigious MJ Awards, recognised in the Innovation in Reducing Health Inequalities category for their impactful work at the heart of local communities.
HOT delivers face-to-face engagement across the borough, providing free health checks and vaccinations directly to communities experiencing the poorest health outcomes. Through their trusted, in-person approach, the team is helping to reduce health inequalities, boost engagement with NHS services, and signpost residents to further free support where it’s needed most.
The team is funded by the NWL Inequalities Transformation and aims to change the way we engage with residents as a Borough Based Partnership. They use population health data to understand where our residents with the greatest need and inequalities are and go to them to have conversations, build trust, and understand their experiences. They work with all organisations within BBP to help bring services into the community, either at the Hounslow Treaty Centre Shop or via the (HOT) community bus, visiting various pockets of the borough.

The team’s community-first approach has helped improve trust in health services and provided a lifeline to many residents across Hounslow. Their efforts can be seen in action in this video.
