Hounslow BBP Celebrates Success as Finalists at HSJ Awards 2025

The Hounslow Borough Based Partnership (BBP) is proud to celebrate the achievement of being named a finalist in the Transforming Care for Older People Award at the 2025 HSJ Awards for its Hounslow Bridging Care Pentagon Model.

While the team did not take home the top award, reaching the finals of one of the most competitive categories is a significant national recognition. It highlights the outstanding collaboration, innovation and commitment demonstrated across health and social care teams in Hounslow.

The shortlisting acknowledges the meaningful impact of the Hounslow Bridging Care programme, which uses the pioneering Pentagon Model to transform support for older patients ready to leave hospital but needing short-term home-based care (Pathway 1).

 The model has delivered measurable improvements, including:

  • ​​​​​​18% reduction in Pathway 1 delay days
  • 64% of patients aged 65+ discharged within 12 working hours
  • Estimated savings of £248 per discharge (2024/25 data)

By bringing together five key services and empowering frontline staff to make real-time decisions, the programme has improved patient flow, enhanced personalised care, and enabled older residents to return home sooner and more safely.

The Pentagon Model brings together services from West London NHS Trust and the London Borough of Hounslow, including:

  • Discharge Hub
  • Reablement Service
  • Home First Team
  • Integrated Community Response Service (ICRS)
  • Hospital Social Work

 Dr Christopher Hilton, Chief Operating Officer (Local Services) at West London NHS Trust and BBP Lead Director, said:

The HSJ Awards are the largest celebration of excellence in UK healthcare, and being named a finalist places the Hounslow Bridging Care, Pentagon Model among the most impactful and innovative programmes nationally.
