Hounslow Bridging Care – Pentagon Model Shortlisted for Prestigious 2025 HSJ Award

The Hounslow Borough Based Partnership (BBP) is proud to announce that its Hounslow Bridging Care – Pentagon Model has been shortlisted for the Transforming Care for Older People Award at the 2025 HSJ Awards.

pexels-rdne-6129159.jpgThis national recognition shines a spotlight on the outstanding collaboration, innovation, and commitment shown by multiple teams working across health and social care in Hounslow.

It is a testament to the BBP’s dedication to delivering healthcare excellence and improving outcomes for local residents.

 

Underpinned by the pioneering “Pentagon Model”, the Hounslow Bridging Care approach has delivered measurable improvements for older patients on Pathway 1 discharges (those who are medically ready to leave hospital but require short-term support at home).

Key outcomes of the model include:

  • 18% reduction in Pathway 1 delay days
  • 64% of patients aged 65 plus discharged within 12 working hours
  • Estimated savings of £248 per discharge (2024/25 data)

By integrating services and empowering frontline practitioners with the autonomy to make real-time decisions, the model has streamlined discharge processes, supported personalised care, and enabled older residents to return home sooner with the right support in place.

Centred in West Middlesex University Hospital, the Pentagon Model brings together five key services across West London NHS Trust and the London Borough of Hounslow, and supports transition of care back to primary care and neighbourhood health teams:

  • Discharge Hub
  • Reablement Service
  • Home First Team
  • Integrated Community Response Service (ICRS)
  • Hospital Social Work

The winners of the 2025 HSJ Awards will be announced at a ceremony on 20 November 2025 at Evolution London. The awards celebrate outstanding achievements in healthcare, promote best practice, and recognise the people and projects making a real difference to patients across the UK.
